Notice of Intent to Suspend
This screen applies to licensed minors who received a Notice of Intent to Suspend/Withhold
Eligibility for Licensure letter from the Department of Highway Safety & Motor
Vehicles (DHSMV).
Your driving privilege may be suspended. A public school district reported you to
DHSMV for non-compliance with school attendance requirements.
If your non-attendance was reported in error, contact the attendance office at the
public school that reported you to DHSMV and request a status letter. The status
letter must be on school letterhead and state that you should not have been reported.
The letter should also include name, date of birth, social security number (if available)
and driver license/control number. The letter can be faxed to
(850) 617-5158 . The letter will serve as authorization for your record
to be amended. Private schools cannot issue status letters.
If you had an attendance problem, please contact the attendance office and request
HSMV Form 72870 . This form will be issued when attendance requirements
are met. Diplomas are also acceptable. The compliance date on the form cannot be
more than 30 days old. The form can be faxed to (850)

If you are:
- Home schooled,
- Enrolled in Job Corps,
- A current out-of-state resident,
- Have been granted a certificate of exemption by a district school superintendent,
or
- Apply for a hardship waiver,

If you are 18 years of age or older school attendance requirements are no longer
applicable. You may apply for a license at your local
driver license office.
